Transcripts & Academic Records

Official transcripts for alumni and students are issued only upon request and are sent directly to another educational institution, state agency, or potential employer. To order a transcript, go to the Office of the University Registrar's website and click on the link for Transcripts and follow the Parchment link. The cost per transcript is $8.25, which includes a Parchment processing fee. The School’s Office of Student Records is no longer able to process requests for transcripts.

Enrollment Verifications

Students and alumni seeking enrollment verifications can obtain them via the National Student Clearinghouse, accessible on the Temple University Registrar’s website.

Other Document Requests

Current medical students and medical alumni requesting documents other than transcripts of enrollment verifications should complete the form (PDF). 

Signed forms may be faxed to 215-707-2940 or dropped off or mailed to:
